Before completing the Team Roster Form, please be sure you have all pertinent information including the following:

• NAGAAA rating for each player
• Your League Commissioner's name
• Your League Commissioner's phone number
• Your League Commissioner's email address

Please note that player ratings will be verified with your league's Commissioner to ensure all information is accurate.
